Patient Registration & Practice Information

All office visits are scheduled by the receptionist. Please call the office to schedule an appointment. Both inpatient and outpatient surgical procedures are performed at Memorial Hospital of Union County, Mary Rutan Hospital, and the Marysville Ohio Surgical Center. Should you need to cancel an appointment, please call as early as possible so we can offer that time to another patient.

Please bring the following items with you to your first appointment:

• Completed patient registration forms
• Your driver’s license
• Your insurance card or other health coverage information
• PCP referral
• X-rays, MRIs and/or test results
• Referring physician notes that relate to your orthopedic condition
• Worker’s compensation claim or case number and appropriate contact information (if applicable)
